Your street address, unless the house runs on a private well, in which case say that instead. The size of your household. Where the water line enters and whether an old softener is next to it. Lab results for the well, if you have any.
Unsure about your supply? If the City of Oconomowoc sends you a water bill, you are on city water.
Who you are dealing with
ZeroGrain Water LLC, a Wisconsin company, does business here under the trade name Oconomowoc Water Softener Co. We work out the size, supply the equipment and line up the installation, which a licensed Wisconsin plumbing contractor performs.
What you buy is one package: whole-home softener, brine tank, and an RO drinking faucet at the kitchen sink. Hardness is measured before the plumber starts and again afterward at your tap, and the work ends at a reading of zero.
Why nobody comes to look first
The price comes from what you tell us, from Oconomowoc's published water figures and, for well homes, from your lab results, so there is nothing to inspect before you decide. The plumber's visit on install day is the only one.
